Rivaroxaban is contraindicated in the following situation: 1. Patients concomitantly on strong inhibitors of both CYP 3A4 and P-glycoprotein such as  HIV protease inhibitors or systemically administered azole antimycotics. 2. Patients undergoing dialysis or patients with severe renal impairment with a creatinine clearance less than 30 mL/min for Rivaroxaban 15 mg and 20 mg tablets and CrCl less than 15 mL/min for Rivaroxaban 10 mg due to increased plasma levels which may lead to an increased risk of bleeding. 3.
